Virtual access to your dedicated provider

Virtual access to your dedicated provider

Primary care, when and where you need it

You don’t have to coordinate your own care. With virtual primary care through Premise Health, you have access to a dedicated provider by phone or video. It’s just like seeing a primary care provider (PCP) in person, but without the waiting room, commute or disruption to your day.

Skip the various appointments at different offices and trying to communicate with multiple providers. Instead, designate one of our virtual primary care providers as your PCP. They’ll become your trusted health ally and help you better prioritize your health.

Virtual visits with the same provider

When you use virtual primary care, your dedicated provider develops a trusting relationship with you. We are not only treating the symptoms that caused you to request a visit, but also create a personalized care plan specific for you, so we can best monitor your health on a consistent basis.

Plus 24/7 access to care when and where you need it

In addition, you have on-demand access to a team of providers who are available 24/7. They understand your health history and communicate with your PCP. This way, you have a full virtual care team dedicated to your well-being.

While virtual primary care may work for many of your health needs, there may be instances where you need in-person care. If this happens, your virtual primary care team is here to get you to the right in-network care in your community.
