Primary care

We offer a comprehensive range of primary care services that focus on your long-term health, including sick and preventive care and management of your conditions. Your primary care provider will get to know you so they are better equipped to prevent, treat and manage any health issues that may come up.

Our lifestyle medicine approach ensures we’re looking at the whole picture of what impacts your wellbeing, including sleep, nutrition, stress, movement and more. We can encourage healthy habits to help you achieve sustainable wellness and go beyond simply treating the symptoms you bring with you to a single appointment.

Treatment and management of health conditions

You don’t have to navigate your health challenges alone. Our providers and registered nurses can be your go-to resource for all health-related questions, prescription guidance and more. We can come up with personalized treatment plans to help you make positive lifestyle changes to prevent, improve, manage or even reverse certain health issues.

Our treatment plans are tailored to meet your unique needs and can address a range of conditions, such as asthma, high blood pressure, obesity, diabetes and pre-diabetes, pregnancy and more. We can provide one-on-one coaching, disease-specific education, self-care training, nutrition counseling, physical activity goals, medication guidance and ongoing monitoring to manage your symptoms and help you stay well.

Behavioral health

We believe that your mental and emotional wellbeing are just as important as your physical health. With our behavioral health services, you can get the support you need. Our licensed clinical social worker actively works as part of your care team to support your overall health, while making you feel safe and comfortable. You can conveniently meet with them in person or virtually, through video or by phone.

With a blended approach of counseling techniques and evidence-based talk therapy, our counseling services cover anxiety, depression, stress management, PTSD, ADHD, grief, adjustment challenges, substance abuse, work/life balance, family conflict and sleep issues.

Women's health

Along with general primary care services, we also offer the following services for women’s health and reproductive wellbeing:

Breast exams
Pelvic exams
Cervical cancer screenings (Pap smears)
Pregnancy testing
Contraception (birth control) counseling
STD testing and counseling
LGBTQ+ care
Hormone therapy
Education for self-awareness

Lab services

We offer many lab services to support your needs. We can perform onsite testing and can also collect and send samples to an offsite lab. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider.

We can perform and process the following with immediate results:

Flu
Rapid strep
Rapid mono
Finger stick glucose
Pregnancy testing
Urine screenings
H-Pylori testing

Our onsite Quest Lab technicians can also perform routine labs such as serum glucose, lipid testing, thyroid testing and Hemoglobin A1, with results back to your provider in 72 hours or less.

Vaccines

Vaccines are an essential part of public health. You have convenient access to many vaccines at the Center for Living Well. Your provider or pharmacist can determine which ones are right for you based on your age, conditions, travel habits and other factors.

We offer you a variety of routine vaccines to protect your health and community, including:

Influenza (Flu)
Hepatitis A
Hepatitis B
Human Papilloma Virus (HPV)
Measles, Mumps and Rubella (MMR)
Pneumococcal (Pneumonia, Meningitis, etc.)
Tdap/Td (Tetanus, Diphtheria and Pertussis / Whooping Cough)
Varicella (Chickenpox)
Zoster (Shingles)

Pharmacy

The Center for Living Well Pharmacy is a convenient way to fill your prescriptions. Our pharmacists are an extension of your care team, working with your primary care provider as needed to support your overall health. We not only fill your prescriptions, we take time to get to know you so we can help you manage your conditions and reach your health goals.

Vision

Good eye health is vital to your overall wellness, which is why we offer a full range of vision care services at the Center for Living Well.* We can meet all your eye-care needs with our state-of-the-art equipment in one convenient location. Our services include:

Comprehensive eye exams
Color vision exams
Foreign body removal
Glaucoma care
Visual field testing
Diagnosing, managing and treatment of ocular disease
Lens fittings

We offer a wide range of Disneylook frames from name brand manufacturers, and a broad selection of lenses, including special designs and enhancements.
